How Tadalafil Daily works
Tadalafil blocks the enzyme PDE5, which normally breaks down cGMP, a messenger that relaxes smooth muscle in penile blood vessels. With more cGMP available, blood flows in more easily when you are sexually aroused. Because tadalafil has a long half-life, a low daily dose keeps a steady level in your body. It does not cause an erection without arousal and does not raise desire.
Who it’s for, and who it isn’t

Usually not a fit
- Anyone using a nitrate in any form, including nitroglycerin tablets or spray, isosorbide, or 'poppers'
- Anyone taking riociguat for pulmonary hypertension
- Men told by a doctor to avoid sexual activity because of heart disease
- Men with a past episode of sudden vision loss from NAION, or severe kidney or liver impairment, without specialist input
- Men already taking another PDE5 inhibitor such as sildenafil or vardenafil

What the evidence says
Tadalafil is FDA-approved for erectile dysfunction, for the signs and symptoms of benign prostatic hyperplasia, and for both together. Once-daily dosing was studied in randomized, placebo-controlled trials that supported its approval. The American Urological Association recommends that men with ED be told about FDA-approved PDE5 inhibitors unless they are contraindicated.
What daily tadalafil is
Tadalafil is the active ingredient in Cialis and its generics. It is a PDE5 inhibitor, the same class as sildenafil, and it is approved by the FDA to treat erectile dysfunction (ED) and urinary symptoms of an enlarged prostate.
Most people know the larger, as-needed doses taken before sex. The daily option uses a smaller dose taken at about the same time every day. Because tadalafil lingers in the body, the level stays fairly steady, and you can respond to arousal at any point in the day.
How the daily dose is set
Your clinician chooses the dose after reviewing your health history, blood pressure and medications. Per the FDA label, once-daily dosing for ED usually starts at 2.5 mg and may be raised to 5 mg depending on how well it works and how you tolerate it. For enlarged-prostate symptoms, or ED and BPH together, the labeled daily dose is 5 mg.
You can take it with or without food. It often takes several days of daily use to reach a steady level, so judge it over a few weeks rather than a single night.
Daily or as-needed: which suits you
Neither approach is better for everyone. Daily dosing appeals to men who want spontaneity or have sex several times a week. As-needed dosing can make more sense if sex is occasional, since you take medicine only when you plan to use it.
If you also wake at night to urinate or have a weak stream from an enlarged prostate, the daily dose can address both concerns with one tablet. Your clinician will weigh this with you.
The nitrate rule and other interactions
Combining tadalafil with any nitrate can drop blood pressure to dangerous levels. That includes nitroglycerin under the tongue or as a spray or patch, isosorbide medicines for angina, and recreational amyl nitrite. The FDA label lists this as a contraindication, whether the nitrate is used daily or only now and then.
Alpha blockers, used for blood pressure or prostate symptoms, also lower blood pressure and need careful handling. Some antifungals, HIV medicines and other drugs raise tadalafil levels. Large amounts of alcohol add to dizziness and low blood pressure. Tell your clinician about everything you take, including supplements.
Why ED can be a heart health signal
Erection problems often reflect the health of small blood vessels. The American Urological Association advises counseling men that ED is a risk marker for cardiovascular disease, and that it may show up years before a heart problem.
For that reason, your assessment asks about blood pressure, cholesterol, blood sugar, smoking and exercise tolerance. The AUA also recommends checking morning testosterone in men with ED, and HealifyMD offers lab testing for that at a separate price.
What it will not do
Tadalafil improves the physical response to arousal. It does not fix relationship strain, performance worry, low desire or fatigue, and it does not treat the underlying causes of vascular disease. It also offers no protection against pregnancy or sexually transmitted infections.
If erections changed suddenly, if you have new chest pain or shortness of breath with exertion, or if you have symptoms such as pain or curvature with erections, your clinician may recommend an in-person evaluation before or alongside treatment. Treating the symptom should never hide a problem that needs attention.
Side effects to know about
The most common side effects in the FDA label are headache, indigestion, back pain, muscle aches, nasal congestion and flushing. They are usually mild and often ease after the first week or two.

Tadalafil Daily: common questions
Is daily tadalafil the same drug as Cialis?
Yes. Generic tadalafil contains the same active ingredient as Cialis and is FDA-approved; the daily form simply uses a lower dose taken every day.
How soon will I notice a difference?
Some men notice an effect within days, but steady levels build over about a week of daily use. Give it a few weeks before deciding with your clinician whether to adjust.
Will tadalafil increase my sex drive?
No. It helps blood flow when you are already aroused; it does not create desire. If low libido is the main issue, your clinician may look at hormones, sleep, mood or medications.
Can I take it if I have high blood pressure?
Often yes, if your blood pressure is controlled and you are not on nitrates. Some blood pressure medicines, especially alpha blockers, need extra care, so your clinician reviews the full list.
Does it help prostate symptoms?
Tadalafil 5 mg daily is FDA-approved for the urinary symptoms of benign prostatic hyperplasia, such as frequent urination and a weak stream.
What if I miss a dose?
Take it when you remember that day, but do not take two doses in one day. Ask your clinician if you are unsure.
What if it doesn't work for me?
Your clinician may adjust the dose, check how you are taking it, or look for other causes such as low testosterone or vascular disease. Other treatment options can be discussed.
