Weight loss

Foundayo pills with a licensed clinician

Foundayo (orforglipron) is Eli Lilly's once-daily weight-loss pill, approved by the FDA in April 2026. Unlike semaglutide tablets, it can be taken at any time of day, with or without food. HealifyMD provides the clinician; LillyDirect bills and ships the medicine.

How Foundayo Pills works

Book a live visit with a licensed clinician. If Foundayo fits your health and goals, your prescription goes directly to LillyDirect, which bills you for the medicine and delivers it to your home. Your HealifyMD program fee covers the clinical care.

Who it’s for, and who it isn’t

May be a fit

  • Adults with a BMI of 30 or more, or 27 or more with a weight-related condition
  • People who would rather take a pill than inject
  • People who find the empty-stomach routine of semaglutide tablets hard to keep up
  • People who want an FDA-approved brand-name medicine

Usually not a fit

  • Pregnancy, breastfeeding or plans to conceive soon
  • Anyone already taking another GLP-1 or tirzepatide product
  • A history of pancreatitis or severe gastrointestinal disease, unless your clinician advises otherwise
  • Anyone under 18

What’s included

  • Live visit with a licensed clinician
  • A Foundayo prescription sent to LillyDirect, only if it is medically appropriate
  • Monthly dose-step plan
  • Follow-up for side effects and dose questions
  • Subscription you can cancel anytime

Possible side effects

  • Common: nausea, diarrhea, vomiting, constipation, stomach pain, indigestion and reduced appetite, mostly while doses are rising
  • Less common but serious: pancreatitis, gallbladder problems, kidney injury from dehydration and low blood sugar when combined with insulin or a sulfonylurea
  • Seek urgent care for severe stomach pain that does not go away, signs of an allergic reaction or trouble swallowing
  • Some medicines, such as oral ketoconazole or clarithromycin, raise orforglipron levels, so your clinician checks your medication list

This isn’t a complete list. Your clinician reviews risks, interactions and warnings with you before prescribing. In an emergency, call 911.

What the evidence says

Foundayo's approval rested largely on the 72-week ATTAIN-1 trial in adults with obesity or overweight, where average weight loss among people who stayed on treatment was 7.8%, 9.3% and 12.4% across the three doses tested. Mild to moderate stomach side effects such as nausea were the most common.

Program fee and medicine cost

The HealifyMD program is $75 a month, or $150 for 3 months ($50 a month), for your clinician visit, the prescription if you qualify and follow-up.

Foundayo is paid for separately to LillyDirect. If prescribed, your prescription is sent directly to LillyDirect for billing and home delivery. Current self-pay prices:

  • $149/month: 0.8 mg and 2.5 mg
  • $199/month: 5.5 mg
  • $299/month: 9 mg
  • $299/month: 14.5 mg and 17.2 mg

A different kind of GLP-1 pill

Semaglutide is a peptide that the stomach breaks down, so the Wegovy and Rybelsus tablets need a strict routine: an empty stomach, a small sip of water and a 30-minute wait. Orforglipron is a small molecule built to be swallowed. It acts on the same GLP-1 receptor but survives digestion, so Foundayo has no food or water rules.

How the doses build up

Foundayo comes in six strengths, from 0.8 mg to 17.2 mg. You start on 0.8 mg once a day for at least 30 days, then step up about every 30 days through 2.5 mg and 5.5 mg toward a maintenance dose between 5.5 mg and 17.2 mg.

That is why the first months cost less: the two starting strengths are $149 a month. Your clinician decides how far to go based on how you respond and how well you tolerate each step.

How it compares with injections

Average weight loss in Foundayo's main trial was lower than in the trials of injectable Wegovy or Zepbound. For many people the trade is worth it: no needles, no refrigeration and no morning routine. Your clinician can compare the options with you, including the Wegovy pill.

Safety points to know first

Nausea, vomiting and diarrhea are the most common side effects, mostly while the dose is rising. Drink enough fluids and tell your clinician if stomach problems last. Ask about the specific warnings on the Foundayo label, and never combine it with another GLP-1 medicine.

Prescription treatments are available only to eligible patients after a clinical evaluation by an independently licensed clinician.
