What we measure before choosing a medicine
A comprehensive lab panel is $199, one time. Your clinician tailors the tests to you.
- Total testosterone on two separate early-morning tests, with SHBG and free testosterone where relevant, and LH and FSH to understand the cause
- HbA1c and fasting glucose, since central weight gain drives insulin resistance
- Lipids with apoB and Lp(a), liver enzymes and kidney function
- Thyroid function, which becomes more often abnormal with age
- Hematocrit and PSA if testosterone therapy might be considered
- Screening questions for sleep apnea and alcohol intake
How weight and testosterone affect each other
A review in the Journal of Clinical Endocrinology & Metabolism describes functional hypogonadism: low testosterone in men over 50 without disease of the testes or pituitary, often linked to obesity, other health conditions or medicines such as opioids. Obesity is the strongest risk factor, and belly fat in particular is linked to symptoms.
The same review reports that diet-related weight loss of about 10% raised testosterone by roughly 2.9 nmol/L on average, with larger rises after surgical weight loss. Weight loss also improved erectile function in a randomized trial. The authors recommend lifestyle measures as first-line care, with testosterone therapy reserved for men with clear deficiency, started alongside or after a lifestyle trial.
Sleep apnea complicates the picture. Excess weight is a leading risk factor for it, and untreated sleep apnea can lower testosterone and raise blood pressure. If your screening suggests it, we refer you for a sleep study.
When low testosterone is treated first, or alongside
Some men have significant symptoms and clearly low levels on repeat testing. For them, testosterone therapy may be reasonable alongside weight loss, especially when low energy and motivation make change harder. TRT needs monitoring of hematocrit and PSA, and it suppresses sperm production, so men who want children soon need a different approach.
For others, especially with borderline results, the better plan is to lose weight, treat sleep problems, cut back on alcohol and retest. Many men find their numbers improve.
GLP-1 medicines: what to expect
Semaglutide and tirzepatide reduce appetite and help you feel full sooner. According to the NIDDK, people using weight-management medicine with lifestyle changes lose on average 3% to 12% more of their starting weight over a year than with lifestyle changes alone. Most people regain some weight after stopping.
Common side effects are nausea, diarrhea, vomiting, constipation and abdominal pain, particularly while the dose is rising. Less common but serious risks include pancreatitis, gallbladder disease and kidney injury from dehydration. These medicines are not used by anyone with a personal or family history of medullary thyroid carcinoma or MEN2 because of a boxed warning about thyroid C-cell tumors in rodents.
HealifyMD's semaglutide and tirzepatide are compounded. Compounded medications are not FDA-approved, and the FDA does not review them for safety, effectiveness or quality. They are not the same as Wegovy or Zepbound.
Protecting muscle while the weight comes off
Losing weight quickly without protecting muscle can leave you lighter but weaker. Federal activity guidelines call for muscle-strengthening work at least two days a week, plus 150 to 300 minutes of moderate aerobic activity. On a GLP-1, eating enough protein matters because appetite drops.
Track waist size and strength, not just weight. If your lifts are falling sharply, tell your clinician so the pace can be adjusted.
What the first three months look like
After a free eligibility check, you complete labs and a video visit. If a GLP-1 is appropriate, your clinician starts a low dose and raises it gradually to limit side effects, checking in by message along the way. If testosterone needs confirming, the second morning test happens early on.
Around three months, you repeat key labs and compare them with your baseline: weight, waist, HbA1c, lipids, liver enzymes and, where relevant, testosterone. The NIDDK notes that if you have not lost at least 5% of your weight after 12 weeks on a full dose, a different approach is usually advised.
Who this suits, and who it does not
This approach suits men over 40 with weight-related health risks who want a measured, clinician-led plan. It is not right for men with a history of medullary thyroid cancer or MEN2, a history of pancreatitis, certain gallbladder or kidney problems, or an active eating disorder. If you are on insulin or a sulfonylurea for diabetes, your doses may need adjusting with your own doctor.

Common questions
Is a GLP-1 different for men than for women?
The medicine works the same way. In men, the plan pays particular attention to testosterone, sleep apnea and keeping muscle.
Will losing weight raise my testosterone?
Often, if excess weight is part of the cause. A meta-analysis found that about 10% diet-related weight loss raised testosterone on average.
Can I be on TRT and a GLP-1 at the same time?
Often yes, if each is appropriate. Your clinician monitors both, including hematocrit and PSA for TRT.
Why is belly fat after 50 so stubborn in men?
Lower muscle mass, less activity, poorer sleep, alcohol and insulin resistance all favor fat storage around the middle.
How much muscle will I lose?
That varies. Strength training, adequate protein and a sensible pace of loss help protect it.
Do I have to do the labs?
Yes. Labs guide safe prescribing and show whether testosterone, blood sugar or other issues need attention.
Are your GLP-1s the same as Wegovy or Zepbound?
No. They are compounded, not FDA-approved and not interchangeable with branded products.
