How the two schedules work
Tadalafil has a long half-life of about 17.5 hours, according to its FDA label. That long tail is what makes both schedules possible. A single as-needed dose was shown to improve erections for up to 36 hours, which is why it earned the "weekend pill" nickname.
Daily dosing uses a much smaller amount taken at roughly the same time each day. Because each dose overlaps the last, blood levels build up and reach a steady state within about five days. At that point the medicine is simply present, and timing around sex stops being a consideration.
What the label says about doses
- As-needed for ED: 10 mg before anticipated sexual activity, adjusted to 5 mg or 20 mg based on response and tolerance, no more than once a day.
- Daily for ED: 2.5 mg at about the same time each day, which may be increased to 5 mg.
- Daily for urinary symptoms of an enlarged prostate, or for ED plus those symptoms: 5 mg once a day.
- Lower maximum doses apply if you take strong CYP3A4 inhibitors such as ketoconazole, itraconazole or ritonavir, or have kidney or liver problems.
Who tends to prefer daily
Daily dosing suits men who have sex more than a couple of times a week, who want spontaneity without counting hours or who find that planning a dose adds pressure. Some men with performance anxiety find that not having to think about a pill helps in its own right.
It is also the obvious choice if you have bothersome urinary symptoms from an enlarged prostate, since the 5 mg daily dose is approved for that too. One medicine can address both problems.
Who tends to prefer as-needed
If sex is occasional, taking a medicine every day may feel unnecessary. As-needed dosing means less total medicine over a month and fewer days with any side effects. It also lets you skip doses around heavy drinking or other situations where you would rather not take it.
Some men also simply want a stronger effect on specific occasions than a daily dose provides. That is a reasonable preference to discuss.
How well each works
Both have solid evidence. In a 12-week trial on the Cialis label, men taking 5 mg daily reported successful intercourse in 46% of attempts versus 13% on placebo. In a 24-week U.S. trial, daily 2.5 mg and 5 mg both improved erectile function scores substantially more than placebo, and a separate trial showed benefit in men with diabetes.
Neither schedule creates arousal. Tadalafil amplifies the natural response to sexual stimulation, so desire, stress, relationship factors and alcohol still matter.
Questions that help you decide
- How often do you have sex, and how predictable is it?
- Does planning around a pill add pressure or feel fine?
- Do you also have urinary symptoms such as getting up at night or a weak stream?
- Did you have side effects with a larger dose that a smaller daily dose might avoid?
- Do you take other medicines, such as alpha-blockers or antifungals, that change how much tadalafil is safe?
- Would you rather take something every day or only occasionally?
Side effects on each schedule
The most common side effects are headache, indigestion, back pain and muscle aches, plus flushing and nasal congestion. On the label, they were more frequent at 20 mg as-needed (headache 15%, indigestion 10%) than at 5 mg daily (headache about 4%, indigestion about 2%), although daily users take the medicine more often.
Rare but serious problems include sudden vision loss, sudden hearing loss and an erection lasting more than four hours; each needs urgent care. Heavy drinking (five or more units) with tadalafil increases dizziness and blood pressure drops on standing.
Rules that apply to both
- Never with nitrates, including nitroglycerin, isosorbide or poppers. If a nitrate is needed in an emergency, at least 48 hours should pass after the last dose.
- Extra caution with alpha-blockers such as doxazosin or tamsulosin.
- Not recommended within 90 days of a heart attack, within six months of a stroke, or with unstable angina, low blood pressure or uncontrolled high blood pressure.
Switching and pricing
You can move between schedules, with your clinician adjusting the dose. If you switch from daily to as-needed, remember that tadalafil lingers for a day or more, so do not stack an as-needed dose on top of a recent daily one without guidance.
HealifyMD offers daily tadalafil from $99/month, confirmed at the eligibility review, with no membership fee. Tadalafil is an FDA-approved medicine available as a generic.
Common questions
Is daily tadalafil better than as-needed?
Neither is better overall. Daily suits frequent sex, spontaneity or prostate symptoms; as-needed suits occasional use.
How long does it take daily tadalafil to work?
Levels reach a steady state within about five days, so judge it after at least a week or two of consistent use.
Can I take an extra dose on top of my daily tadalafil?
Not without your clinician's advice. Tadalafil lasts a long time and extra doses raise the risk of side effects.
What if I miss a daily dose?
Take it when you remember that day, but do not double up the next day. Your level will usually stay fairly steady.
Does daily tadalafil help with urination?
The 5 mg daily dose is FDA-approved for urinary symptoms of an enlarged prostate.
Are side effects worse with daily use?
On the label, side effects were less frequent at the low daily dose than at 20 mg as-needed, though they can still occur.
Can I drink alcohol with tadalafil?
Moderate drinking is usually acceptable, but heavy drinking increases dizziness and blood pressure drops.
