How to use these guides
Start with the symptom you actually noticed, not the diagnosis you suspect. Each guide explains what the symptom usually means in midlife, the hormonal and non-hormonal causes worth considering, the warning signs that need urgent attention, what testing can and cannot show, and the general treatment options.
Many midlife symptoms overlap. Poor sleep, low iron, thyroid problems, medications and mood changes can mimic hormonal shifts, and hormonal shifts can make other conditions feel worse. That is why every guide lists non-hormonal causes alongside hormonal ones, and why none of them assumes hormones are to blame.
Symptoms in perimenopause and menopause
These symptoms are common during the menopause transition, though each also has other possible causes:
- Hot flashes and the heat, flushing and sweating that come with them
- Frozen shoulder and joint pain in menopause
- Perimenopause headaches and migraine changes
- Heart palpitations, anxiety and irritability
- Breast tenderness, bloating and nausea
- Itchy skin, acne and dry eyes
- Vaginal dryness and painful sex
- Menopause belly and changes in body shape
- Hair loss and thinning
- Spotting, irregular bleeding and heavy periods
Symptoms in men
Some symptoms are specific to men or carry different meaning in men:
- Hot flashes in men, often linked to a sudden drop in testosterone
- Gynecomastia, or enlarged breast tissue
- Loss of morning erections, which can be an early signal of blood vessel, hormone or sleep problems
Symptoms in anyone
These symptoms affect women and men, and their causes are often metabolic, nutritional, sleep-related or neurological rather than hormonal:
- Night sweats
- Brain fog
- Snoring
- Restless legs
- Always feeling cold
- Brittle nails
- Tingling in the hands and feet
- Waking up to urinate at night
- Lack of motivation
Red flags come first
Every guide lists the warning signs that mean you should seek in-person or emergency care before anything else. Some apply across many symptoms. Call 911 for chest pain or pressure, sudden weakness or numbness on one side, trouble speaking, a sudden severe headache, fainting, or severe shortness of breath. Call or text 988 if you are having thoughts of harming yourself.
Seek prompt in-person evaluation for unexplained weight loss, recurring fevers, a new lump, any vaginal bleeding after 12 months without a period, or symptoms that are rapidly getting worse. Online care is not the right first step in those situations.
What testing can and cannot tell you
Blood tests are very good at finding some causes of midlife symptoms, such as thyroid disease, iron deficiency, low vitamin B12, high blood sugar and low testosterone in men. They are much less useful for diagnosing perimenopause, because estrogen and FSH fluctuate from week to week; the transition is recognized mainly from your age, cycle changes and symptoms.
Other problems need different tools entirely. Sleep apnea needs a sleep study, heart rhythm problems need an ECG, breast and pelvic concerns need an exam and imaging, and joint, nerve and skin problems usually need hands-on examination. The guides say plainly when that is the case.
How HealifyMD fits in
HealifyMD is a telehealth service for adults in midlife. Through HealifyMD, independently licensed clinicians can review your symptoms online, order lab testing, and, where appropriate, prescribe treatments such as menopausal hormone therapy, testosterone therapy, medical weight management and sexual health medications, with medication shipped from licensed U.S. pharmacies. The eligibility check is free and there is no membership fee; labs are priced separately and prices are confirmed at the eligibility review.
We are also clear about our limits. We do not examine patients in person, perform imaging, run sleep studies or manage emergencies. When a symptom needs those things, we will tell you and help you find the right in-person care. All symptom guides are reviewed for medical accuracy by Dr. Vanessa Niles, a board-certified OB-GYN.
