Tadalafil daily

Tadalafil or sildenafil: how to choose

Both are FDA-approved PDE5 inhibitors and both work well for most men. The real differences are about timing, how long they last and how they fit your routine.

Tadalafil only (2.5–5 mg)

The same mechanism

Sildenafil and tadalafil both block PDE5, an enzyme that ends an erection by breaking down the chemical signal that keeps penile blood vessels relaxed. With PDE5 blocked, that signal lasts longer when you are aroused. Neither medicine causes an erection without sexual stimulation, and neither increases desire.

Because they work the same way, they share the same key safety rules. The differences that matter in daily life are timing, duration and side effect patterns.

Timing and duration

MedlinePlus advises taking sildenafil for ED about an hour before sexual activity, though anywhere from 30 minutes to four hours beforehand is acceptable, and no more than once a day. Its useful window is a matter of hours.

Tadalafil's FDA label lists a half-life of about 17.5 hours, and as-needed doses improved erections for up to 36 hours. That longer window takes some pressure off timing. Tadalafil is also approved as a small daily dose that keeps a steady level, removing timing from the equation altogether.

Food and alcohol

A high-fat meal delays how quickly sildenafil starts working, so taking it after a big dinner can make it seem weaker. Tadalafil can be taken with or without food.

With either medicine, heavy drinking works against you: alcohol itself impairs erections, and the Cialis label notes that five or more units with tadalafil increase dizziness, headache and blood pressure drops when standing.

Side effects compared

  • Shared: headache, flushing, indigestion, nasal congestion and muscle aches.
  • More typical of sildenafil: brief changes in color vision, such as a blue tinge, and light sensitivity.
  • More typical of tadalafil: back pain and muscle aches, which can start several hours after a dose.
  • Rare but serious with both: sudden vision loss, sudden hearing loss and an erection lasting more than four hours. Each needs urgent care.

Safety rules that apply to both

Never combine either medicine with nitrates, including nitroglycerin, isosorbide, nitrate patches or poppers. The combination can cause a dangerous drop in blood pressure. Because tadalafil lasts longer, its label says at least 48 hours should pass before a nitrate is given in an emergency.

Both need care with alpha-blockers and are not recommended after a recent heart attack or stroke, or with unstable angina, very low blood pressure or uncontrolled high blood pressure. Some medicines, including certain antifungals and HIV drugs, raise tadalafil levels and call for lower maximum doses, so bring a full medication list whichever drug you consider.

What a fair comparison looks like

If you are trying to decide between them based on your own experience, give each a proper chance. That means several attempts at a dose your clinician has adjusted, with arousal, without a heavy meal before sildenafil and without much alcohol. One disappointing night says little about a medicine.

Keep brief notes: when you took it, what you ate and drank, how firm and lasting the erection was and any side effects. That record makes the next conversation with your clinician far more useful than a general impression, and it often shows whether the issue is the drug, the timing or something else entirely.

Other approved uses

Tadalafil 5 mg daily is FDA-approved for urinary symptoms of an enlarged prostate, alone or with ED, which makes it a two-in-one option for many men over 50. Sildenafil does not have that approval. Sildenafil is approved, under other brand names, for pulmonary arterial hypertension, a lung blood vessel condition, at different doses.

How to choose

  • Want to plan less, or have sex more than a couple of times a week: tadalafil, possibly daily.
  • Have urinary symptoms from an enlarged prostate: daily tadalafil.
  • Prefer a shorter-acting medicine used only occasionally: sildenafil is reasonable.
  • Had back aches on tadalafil or vision changes on sildenafil: try the other.
  • One did not work after a fair trial: switching is worth trying, and so is looking for an underlying cause.

At HealifyMD

Your clinician reviews your heart history, blood pressure and medicines before prescribing any PDE5 inhibitor. The AUA recommends informing men about the benefits and risks of each FDA-approved option and adjusting the dose for the best effect, and that is how we approach it. Daily tadalafil is from $99/month, confirmed at the eligibility review, with no membership fee.

Common questions

Is tadalafil stronger than sildenafil?

Not in general. At their usual doses they work similarly well; tadalafil simply lasts longer.

Which one lasts longer?

Tadalafil. Its effects were shown for up to 36 hours, compared with a window of a few hours for sildenafil.

Does food affect them differently?

Yes. A high-fat meal delays sildenafil, while tadalafil can be taken with or without food.

Can I switch from one to the other?

Yes, with your clinician's guidance. Do not take both on the same day.

Does the nitrate warning apply to both?

Yes. Neither can ever be combined with nitrates or poppers.

Does either help an enlarged prostate?

Daily tadalafil is FDA-approved for urinary symptoms of BPH. Sildenafil is not approved for that use.

Which has fewer side effects?

They are similar overall; the pattern differs. Sildenafil more often affects color vision, tadalafil more often causes back aches.
