Enclomiphene / DHEA / boron / pregnenolone ODT

A compounded dissolving tablet built around enclomiphene, a medicine that nudges the brain to signal the testes to make more testosterone. It is an option for some men with low testosterone who want to protect fertility, not a replacement for testosterone therapy in everyone.

How Enclomiphene / DHEA / Boron / Pregnenolone ODT works

Enclomiphene is a selective estrogen receptor modulator. It blocks estrogen's feedback at the hypothalamus and pituitary, so the brain reads estrogen as low and releases more LH and FSH. LH tells the testes to make testosterone, and FSH supports sperm production, which is why enclomiphene can raise testosterone without shutting down fertility the way testosterone therapy can.

Who it’s for, and who it isn’t

May be a fit

  • Men with symptoms of low testosterone and confirmed low morning testosterone levels on repeat testing
  • Men whose labs point to secondary hypogonadism, where the testes work but the brain's signal (LH and FSH) is low
  • Men who want to keep natural sperm production, for example because they may want children
  • Men who prefer to avoid injections or gels and accept closer lab monitoring

Usually not a fit

  • Men with primary hypogonadism, where the testes themselves are not responding
  • Anyone with a history of blood clots or a clotting disorder
  • Men with significant liver disease
  • Men with visual disorders that could be worsened, or who develop vision changes on treatment
  • Men with untreated pituitary disease, prostate or breast cancer
  • Women and anyone under 18

What’s included

  • Clinician review of symptoms, history and medications
  • Baseline and follow-up lab planning (labs priced separately)
  • Medication shipped from a licensed U.S. pharmacy
  • Follow-up to judge response, side effects and lab changes

Possible side effects

  • Headache
  • Hot flushes
  • Mood changes or irritability
  • Nausea
  • Rises in estradiol, sometimes with breast tenderness
  • Visual disturbances such as blurring or spots (stop and call your clinician)
  • DHEA-related acne or oily skin

What the evidence says

Phase III trials in overweight men with secondary hypogonadism found that oral enclomiphene raised testosterone, LH and FSH over 16 weeks while keeping sperm concentration in the normal range, whereas testosterone gel lowered sperm production. Enclomiphene was never FDA-approved, and long-term outcome data are lacking. Evidence that the added DHEA, boron and pregnenolone improve results is limited.

Restoring your own production instead of replacing it

Testosterone therapy supplies the hormone from outside. Your brain senses it and turns down LH and FSH, so the testes make less testosterone and much less sperm. Enclomiphene takes the opposite approach: it turns the brain's signal up, so the testes do more of the work themselves.

That difference is why enclomiphene interests men who want to preserve fertility, or who would rather not depend on external testosterone. It only works if the testes can respond. In primary hypogonadism, where the testes are damaged or failing, raising LH has little effect, and testosterone therapy is usually the better option.

Getting the diagnosis right first

The Endocrine Society recommends diagnosing low testosterone only when symptoms are present and morning fasting testosterone is consistently low on repeat testing. Your clinician will also look at LH, FSH, estradiol, prolactin and other labs to tell primary from secondary causes and to spot problems such as pituitary disease.

Common, fixable contributors include poor sleep, sleep apnea, excess weight, heavy drinking, opioids and some other medicines. Addressing these can raise testosterone on its own and sometimes changes whether medication is needed at all.

What the other ingredients add

This tablet also contains DHEA, boron and pregnenolone. DHEA is an adrenal hormone that the body can convert into testosterone and estrogen; pregnenolone is an upstream building block for many steroid hormones; boron is a trace mineral studied in small trials for effects on hormone levels. The evidence that these additions meaningfully improve symptoms or outcomes is much weaker than for enclomiphene itself, and your clinician will be candid about that.

FDA status and compounding

Enclomiphene is not FDA-approved for any use. It was studied for secondary hypogonadism, but FDA did not approve it. In 2022 an FDA advisory committee voted against adding enclomiphene to the list of bulk substances that pharmacies may compound under section 503A. FDA's nomination list, last updated in 2026, still shows enclomiphene as under evaluation, which is the basis on which pharmacies continue to compound it.

This product is compounded by a licensed U.S. pharmacy. Compounded medications are not FDA-approved, are not reviewed by FDA for safety, effectiveness or quality and are not interchangeable with any branded product. Clomiphene, a related FDA-approved drug for women's fertility, is sometimes used off-label in men as an alternative.

Taking a dissolving tablet

An orally dissolving tablet melts on or under the tongue rather than being swallowed whole. For many men this is simply a convenient way to take a daily medicine without water. Your prescription label sets the dose and schedule, and your clinician may adjust it after the first round of labs.

Take it at roughly the same time each day so blood levels are easier to interpret. Avoid eating or drinking until it has fully dissolved, unless your instructions say otherwise. If you miss a dose, do not double the next one. Keep the tablets dry and away from heat, since moisture can soften them, and let your clinician know if you start or stop any other medicine, supplement or bodybuilding product, because some can push hormone levels in unexpected directions.

Safety and what to watch for

Medicines in this class can affect vision, so any blurring, flashes or spots should prompt you to stop and contact your clinician. SERMs have been linked with blood clots, which is why a clot history rules this out. Estradiol can rise along with testosterone, occasionally causing breast tenderness or mood changes.

  • Sudden vision changes: stop and call
  • Leg swelling, chest pain or sudden breathlessness: emergency care
  • Marked mood swings or irritability: let your clinician know

What your clinician monitors

Expect repeat labs a few weeks after starting and periodically afterward, typically total testosterone, LH, FSH, estradiol, hematocrit and, depending on age, PSA. Your clinician compares the numbers with how you actually feel: energy, libido, mood and sleep. If testosterone rises but symptoms do not change, the plan should be revisited rather than simply continued.

The ODT costs from $129 a month, or $349 for three months ($116/month), with no membership fee. Labs are billed separately, and your price is confirmed at the eligibility review.

Enclomiphene / DHEA / Boron / Pregnenolone ODT: common questions

How is this different from testosterone therapy?

Testosterone therapy replaces the hormone and usually suppresses sperm production. Enclomiphene prompts your own testes to make more, which tends to preserve fertility.

Is enclomiphene FDA-approved?

No. It is not FDA-approved for any use and is available only through compounding pharmacies.

Will it work for me?

It works best in secondary hypogonadism, when the testes can still respond. Labs help your clinician predict that before you start.

How soon might I notice a difference?

Testosterone levels can rise within weeks. Changes in energy or libido, if they come, often take one to three months.

Can I switch from TRT to enclomiphene?

Sometimes. Your own production may take time to recover after stopping testosterone, so the transition needs a clinician-guided plan and labs.

Why include DHEA, boron and pregnenolone?

They are hormone precursors or nutrients thought to support hormone balance, but evidence for added benefit is limited.

Does it affect fertility?

Studies suggest enclomiphene keeps sperm counts in the normal range, unlike testosterone gel. If fertility is a priority, a semen analysis can confirm where you stand.
