Semaglutide injection

Semaglutide and fatty liver disease

Fatty liver is common in midlife and usually silent. One form of it now has an FDA-approved semaglutide treatment, but the approval is narrower than many people assume.

The new names for fatty liver

Doctors now call the condition steatotic liver disease. When it is linked to metabolic factors like excess weight, prediabetes, type 2 diabetes, high triglycerides or high blood pressure, it is MASLD (metabolic dysfunction-associated steatotic liver disease), formerly NAFLD.

MASH (metabolic dysfunction-associated steatohepatitis), formerly NASH, is the more serious form, where fat is joined by inflammation and liver cell damage. Over time MASH can lead to scarring (fibrosis), cirrhosis and liver cancer. Fatty liver caused mainly by heavy drinking is a separate condition, alcohol-associated liver disease.

Why midlife adults should pay attention

MedlinePlus lists middle age or older, obesity, type 2 diabetes or prediabetes, high cholesterol or triglycerides and high blood pressure among the risk factors. Many people with weight gain in their 40s and 50s check several of those boxes.

The disease usually causes few or no symptoms. It is often first suspected from abnormal liver blood tests or an imaging report that mentions fat in the liver, which is why it is easy to overlook.

When symptoms do appear, they tend to be vague: tiredness or a dull discomfort under the right ribs. Signs of advanced disease, such as yellowing of the skin or eyes, swelling in the legs or belly, or easy bruising, need prompt in-person evaluation rather than an online visit.

What the FDA approved

The Wegovy label now includes treatment of noncirrhotic MASH with moderate to advanced fibrosis (stages F2 to F3) in adults, together with a reduced-calorie diet and more physical activity. The supporting trial enrolled 1,195 adults with biopsy-confirmed disease.

This is an accelerated approval. It rests on improvements seen in liver biopsies, and continued approval may depend on a confirmatory trial showing benefit on longer-term outcomes such as progression to cirrhosis. Those outcome results have not yet been reported.

Wegovy was not the first MASH drug. In March 2024 the FDA approved resmetirom (Rezdiffra), also under accelerated approval, for a similar group of patients.

What the approval does not cover

  • Simple fatty liver without inflammation or significant scarring.
  • Cirrhosis. The approval is for noncirrhotic disease only.
  • Wegovy tablets. The MASH indication applies to the injection.
  • Compounded semaglutide. It has not been studied for MASH, is not FDA-approved and has not been reviewed by the FDA for safety, effectiveness or quality.
  • Alcohol-associated liver disease, where stopping alcohol is the key treatment.

How liver scarring is assessed

Staging matters because the approval is tied to fibrosis level. Assessment usually starts with your history, including alcohol and medicines, plus a physical exam and blood tests of liver function and blood counts. Imaging can detect liver fat and estimate stiffness, which reflects scarring. A biopsy is sometimes needed to confirm the diagnosis and measure damage.

HealifyMD clinicians can order blood work, and our comprehensive lab panel is $199. Imaging, specialist referral and any biopsy decisions belong with your primary care doctor or a hepatologist, and we will tell you when your results point there.

Weight loss still comes first

Weight loss is the first-line treatment for MASLD because it can reduce liver fat, inflammation and scarring. A GLP-1 medicine can help some people reach that goal, but it works best as part of a plan that includes food changes, regular movement, limiting alcohol and checking supplements with a clinician, since some can harm the liver.

Semaglutide's usual cautions apply: a boxed warning about thyroid C-cell tumors in rodents, with no use if you or a family member has had medullary thyroid carcinoma or if you have MEN 2; stomach side effects; and risks of pancreatitis, gallbladder disease and kidney injury from dehydration. It is not for use in pregnancy.

Questions to ask about your liver results

  • Do my blood tests or imaging suggest fat only, or inflammation and scarring too?
  • Has my fibrosis stage been estimated, and by which test?
  • Should I see a hepatologist or gastroenterologist before starting a medicine?
  • Could any of my medicines or supplements be contributing?
  • How often should my liver tests be repeated once I start losing weight?

How HealifyMD can help

If you have confirmed MASH with F2 to F3 fibrosis, FDA-approved Wegovy is the evidence-based semaglutide option, and our weight loss membership supports people using branded GLP-1s from $75/month. If your clinician recommends compounded semaglutide for weight management, it is from $299/month, confirmed at the eligibility review, and we will not present it as a liver treatment.

Common questions

Is semaglutide approved for fatty liver?

Wegovy injection is approved for adults with MASH and moderate to advanced fibrosis without cirrhosis. It is not approved for every type of fatty liver.

What is the difference between MASLD and MASH?

MASLD is fat buildup in the liver linked to metabolic factors. MASH is the more severe form with inflammation and cell damage, which can lead to scarring.

Can I use semaglutide if I have cirrhosis?

The MASH approval does not cover cirrhosis. Anyone with cirrhosis needs specialist care before considering any new medicine.

Will compounded semaglutide treat my liver?

It has not been studied for MASH and is not FDA-approved. We do not present compounded semaglutide as a liver treatment.

How do I know if I have scarring?

Blood tests and imaging that estimates liver stiffness are common first steps, and a biopsy is sometimes needed. Your doctor will interpret them together.

Will my liver enzymes return to normal?

They may improve with weight loss and treatment, but responses vary. Normal enzymes do not always mean scarring has resolved, so follow-up testing matters.

Does alcohol matter if my fatty liver is metabolic?

Yes. Alcohol adds stress to the liver, so most clinicians advise cutting back or avoiding it when you have liver disease.
