Semaglutide injection

Compounded semaglutide and Wegovy are not the same product

Both contain semaglutide, but only one has been through FDA review. Here is what that difference means for quality, dosing, legality and your choice.

Same active ingredient, different journey to you

Wegovy reached pharmacy shelves after its maker ran large randomized trials, submitted the data to the FDA and passed review of how the drug is made, labeled and packaged. Its label lists approved uses: lowering the risk of heart attack, stroke and cardiovascular death in adults with established heart disease and excess weight, long-term weight reduction, and (for the injection) a liver condition called MASH with moderate to advanced scarring.

Compounded semaglutide skips that pathway. A licensed pharmacy prepares it from a prescription for one patient. The FDA states plainly that compounded drugs are not FDA-approved and that the agency does not verify their safety, effectiveness or quality before they are sold. The trials behind Wegovy were run on Wegovy, not on any compounded preparation.

Where the practical differences show up

Beyond the approval itself, a few everyday details separate the two. None of them makes semaglutide work differently in your body, but they affect consistency and the chance of mistakes.

  • Device: Wegovy injection comes in a prefilled, single-dose pen set to fixed strengths. Compounded semaglutide is often supplied in a vial that you draw up with a syringe, which leaves more room for measuring errors.
  • Manufacturing oversight: approved drugs are made under federal manufacturing standards. Most compounding happens in state-licensed 503A pharmacies, which are overseen mainly by state pharmacy boards and are not held to those same manufacturing rules.
  • Ingredient form: the FDA says semaglutide sodium and semaglutide acetate are different active ingredients from the one in approved products, and it knows of no lawful basis for compounding with them.
  • Evidence: the weight, heart and liver results on Wegovy's label come from trials of the branded product. They cannot simply be transferred to a compounded version.

Why compounded semaglutide became so common

Federal law lets pharmacies compound versions of a drug that is on the FDA shortage list. Semaglutide injection was in shortage for a long stretch as demand soared, and many compounders filled the gap.

On February 21, 2025, the FDA declared the semaglutide injection shortage resolved. It gave state-licensed pharmacies until April 22, 2025, and outsourcing facilities until May 22, 2025, before ending the flexibility tied to the shortage. Since then, a compounded version is meant for a patient whose prescriber has a documented clinical reason the approved product does not meet, not as a routine copy of Wegovy.

What the FDA has seen with compounded GLP-1s

As of May 31, 2026, the FDA had received 990 adverse event reports involving compounded semaglutide and notes that the true number is probably higher because many pharmacies are not required to report. Some of the reports, including hospitalizations, were linked to dosing errors: patients measuring the wrong amount from a vial, or clinicians miscalculating a dose.

The agency has also warned about counterfeit and falsely labeled products sold online, and about "research use only" peptides being sold for people. These risks are a reason to use only a prescriber and a licensed U.S. pharmacy you can identify.

Safety rules that apply to both

Whatever the source, semaglutide carries the same core cautions. The Wegovy label has a boxed warning: semaglutide caused thyroid C-cell tumors in rodents, and it is unknown whether it does in people. It must not be used if you or a family member has had medullary thyroid carcinoma (MTC) or if you have multiple endocrine neoplasia type 2 (MEN 2).

Nausea, vomiting, diarrhea and constipation are common, especially after dose increases. Less common but serious risks include pancreatitis, gallbladder disease, kidney injury from dehydration and low blood sugar if combined with insulin or sulfonylureas. Semaglutide is not for use in pregnancy, and the label advises stopping at least two months before a planned pregnancy.

How HealifyMD approaches the choice

If an FDA-approved product fits your needs and you can access it, that is a sound starting point, and HealifyMD offers a weight loss membership from $75/month for people who obtain branded GLP-1s through their own coverage. If your clinician believes a compounded preparation is appropriate for you, it is filled by a licensed U.S. pharmacy and clearly identified as compounded.

Compounded semaglutide through HealifyMD is from $299/month, or $550 for 2 months ($275/month). Your price is confirmed during the eligibility review; there is no membership fee for the compounded plan and labs are priced separately. Care is available only in states where our clinicians are licensed.

Questions worth asking any provider

  • Is this product compounded or FDA-approved, and is that written on the label?
  • Which licensed pharmacy prepares and dispenses it?
  • Is the active ingredient semaglutide base rather than a salt form?
  • How will my dose be measured, and who checks my technique?
  • Who do I contact if I have severe side effects?

Common questions

Is compounded semaglutide a generic Wegovy?

No. There is no FDA-approved generic of Wegovy. Compounded semaglutide is a pharmacy preparation that has not been reviewed or approved by the FDA.

Does compounded semaglutide work as well as Wegovy?

That has not been tested. Wegovy's trial results apply to Wegovy, and compounded products have not been studied in comparable trials.

Is compounded semaglutide still legal now that the shortage is over?

State-licensed pharmacies can still compound for an individual patient in limited circumstances, but federal law restricts making routine copies of an available approved drug. Rules continue to evolve, so your clinician and pharmacy must confirm a product can be lawfully dispensed.

What is semaglutide sodium?

It is a salt form of semaglutide. The FDA says salt forms are different active ingredients from approved semaglutide and that it is not aware of a lawful basis for compounding with them.

Can I switch from compounded semaglutide to Wegovy?

Often yes, but doses and strengths may not line up neatly. Your clinician will plan the switch and may restart at a lower dose to limit side effects.

Does insurance pay for compounded semaglutide?

Insurance generally does not cover compounded GLP-1s. Coverage for Wegovy depends on your plan.

Why do the prices differ so much?

Branded drug pricing reflects development, trials and approval costs. A lower price does not mean the products are interchangeable.
