Why swallowing semaglutide is hard
Semaglutide is a peptide, a chain of amino acids that your stomach normally breaks down. Getting enough of it into the bloodstream from a pill requires special formulation. The approved tablets pair semaglutide with an absorption enhancer and come with strict instructions, because even small changes in how you take them affect how much is absorbed.
That is the core issue when comparing products: the molecule is the same, but the amount that reaches your blood depends heavily on the formulation and routine.
Rybelsus: the diabetes tablet
Rybelsus was the first oral GLP-1. It is approved as an add-on to diet and exercise to improve blood sugar control in adults with type 2 diabetes. It is taken once daily, at least 30 minutes before the first food or drink of the day, with doses raised in steps.
People with type 2 diabetes often lose some weight on Rybelsus, but it is not approved for weight management. Using it for weight loss alone would be off-label.
Wegovy tablets: approved for weight
Wegovy now comes as both an injection and a once-daily tablet. The tablet is approved, with diet and exercise, for long-term weight reduction in adults with obesity or with overweight plus a weight-related condition, and to reduce the risk of major cardiovascular events in adults with established heart disease and excess weight. The MASH liver indication applies to the injection only.
The label gives exact rules: one tablet each morning on an empty stomach with up to 4 ounces of plain water, swallowed whole, then wait at least 30 minutes before eating, drinking or taking other oral medicines. A missed dose is skipped, not doubled. The tablet was tested in a 64-week trial of a 25 mg dose, published in the New England Journal of Medicine in 2025.
Compounded capsules: what they are and are not
Compounded oral semaglutide is prepared by a licensed pharmacy for an individual prescription. The formulation, including any absorption aid, is chosen by the pharmacy, and it has not been tested in clinical trials. The FDA states that compounded drugs are not FDA-approved and are not reviewed for safety, effectiveness or quality before they are sold.
That means we cannot tell you how much semaglutide a capsule delivers compared with an approved tablet, and responses may vary more from person to person. Results from the Wegovy tablet trial do not transfer to capsules. Your HealifyMD clinician follows your response closely rather than assuming equivalence.
Side-by-side
- Approval: Rybelsus and Wegovy tablets are FDA-approved; compounded capsules are not.
- Approved use: Rybelsus for type 2 diabetes; Wegovy tablets for weight and heart risk reduction; capsules have no approved use.
- Evidence: both brands have randomized trials; capsules have none.
- Routine: both brands require an empty stomach, a small amount of water and a 30-minute wait; capsule instructions come from your prescriber and pharmacy.
- Insurance: brands may be covered depending on your plan; compounded capsules generally are not.
A non-peptide pill without the ritual
In April 2026 the FDA approved orforglipron (Foundayo), a once-daily GLP-1 pill for adults with obesity, or overweight with weight-related conditions. It is a small molecule rather than a peptide, so it can be taken without the food and water restrictions that apply to semaglutide tablets. It is a different drug from semaglutide, with its own trial data and side effect profile, and your clinician can discuss whether it fits.
Safety that applies to every version
All semaglutide products carry the boxed warning about thyroid C-cell tumors seen in rodents. They are not used by anyone with a personal or family history of medullary thyroid carcinoma or with MEN 2. Nausea, diarrhea, vomiting and constipation are common; pancreatitis, gallbladder disease, low blood sugar with insulin or sulfonylureas and kidney injury from dehydration are serious risks. Semaglutide is not used in pregnancy. Oral semaglutide can raise levothyroxine levels, so tell your clinician if you take thyroid medicine.
How to choose
If you have coverage for an approved tablet and can follow its morning routine, that is the option with the strongest evidence; HealifyMD's weight loss membership supports people on branded GLP-1s from $75/month. If your clinician recommends a compounded capsule, it is $299/month, confirmed at the eligibility review, with no membership fee and labs priced separately. If needles are not a concern, an injection may give more predictable absorption.
Common questions
Is the Wegovy pill the same as Rybelsus?
Both contain semaglutide, but they are different products with different approved uses and doses. Rybelsus is for type 2 diabetes; Wegovy tablets are for weight and heart risk reduction.
Is compounded oral semaglutide as good as the Wegovy pill?
That is unknown. Compounded capsules have not been tested in trials, are not FDA-approved and may be absorbed differently.
Why must oral semaglutide be taken on an empty stomach?
Food, drinks and other pills reduce how much semaglutide is absorbed. The 30-minute wait gives it time to be taken up.
Can I take Rybelsus for weight loss?
Rybelsus is approved only for type 2 diabetes. Wegovy tablets are the semaglutide pill approved for weight management.
Which GLP-1 pill has no food rules?
Orforglipron (Foundayo), a non-peptide pill approved in April 2026, can be taken without food or water restrictions.
Do the tablets carry the same heart benefit as the injection?
The Wegovy label includes cardiovascular risk reduction for both the injection and the tablets. Compounded capsules have no such data.
Can I switch from compounded capsules to a branded tablet?
Yes, with your clinician's plan. Doses do not convert one to one, so you may restart at a lower strength.
