What a troche is
A troche (pronounced "tro-key") is a small, often flavored lozenge made by a compounding pharmacy. You hold it between your cheek and gum or under your tongue until it dissolves, usually over several minutes. Some of the medicine passes through the thin lining of the mouth into the blood; the rest is swallowed with saliva.
Pharmacies use troches when a prescriber wants several ingredients in one dose, or when an ingredient is poorly suited to swallowing. Your HealifyMD prescription label tells you the exact form of your Triple Action Blend and how to use it.
Why peptides are rarely swallowed
The blend combines tadalafil with two peptides, PT-141 (bremelanotide) and oxytocin. Peptides are chains of amino acids, and the digestive system is built to break those down. That is why the only FDA-approved form of bremelanotide, Vyleesi, is an injection under the skin, and why human research on oxytocin and sexual function has mostly used nasal sprays.
Absorption through the mouth lining is one way compounders try to get around this. Whether a meaningful amount of each peptide actually reaches the bloodstream from a troche has not been established in published studies.
What the form can change
- Onset: medicine absorbed through the mouth lining can enter the blood without first passing through the stomach and liver, which may speed onset for some drugs.
- Food: swallowed sildenafil is slowed by a high-fat meal; FDA-approved tadalafil tablets can be taken with or without food. A troche sidesteps the stomach for whatever portion is absorbed in the mouth.
- Convenience: no water is needed, but you have to hold the troche in place without chewing until it dissolves.
- Consistency: absorption depends on how long it stays in your mouth, how much you swallow and your saliva, so it can vary more from dose to dose.
What the form does not change
Tadalafil is still tadalafil. The nitrate rule applies in full: never combine it with nitroglycerin, isosorbide, nitrate patches or poppers, because the combination can cause a dangerous drop in blood pressure. Alpha-blocker cautions, heart condition limits and side effects such as headache, indigestion, back pain, flushing and, rarely, vision or hearing loss or a prolonged erection still apply.
Tadalafil is long-acting, with a half-life of about 17.5 hours on its FDA label. A troche does not shorten that. PT-141's effects, including nausea, flushing and a temporary rise in blood pressure, also do not disappear because of the route.
And the evidence does not change. The trials behind tadalafil used swallowed tablets. No trial has tested this three-ingredient blend in any form.
Side effects to expect with any form
With tadalafil, the label lists headache, indigestion, back pain and muscle aches as the most common side effects; with the lower daily doses they were less frequent. PT-141 frequently causes nausea and flushing, especially with the first doses.
A troche may add minor mouth effects such as an unpleasant taste or irritation where it sits. Stop and get urgent help for sudden vision or hearing loss, chest pain, fainting or an erection lasting more than four hours.
Compounded, not FDA-approved
The Triple Action Blend is prepared by a licensed U.S. compounding pharmacy for an individual prescription. The FDA states that compounded drugs are not FDA-approved and are not reviewed for safety, effectiveness or quality before they are sold. The blend is not equivalent to Cialis, Vyleesi or any branded product.
That does not mean a compounded product is unsafe, but it does mean you should rely on your clinician and pharmacist for instructions rather than general advice online, and tell them about every other medicine you take.
Using a troche well
- Follow the timing on your prescription label.
- Place it where your instructions say, and let it dissolve; do not chew or swallow it whole.
- Avoid eating or drinking until it has fully dissolved, unless told otherwise.
- Never take an extra dose because the first seems slow; tadalafil builds and lasts.
- Tell your clinician if holding it in your mouth is difficult, or if you have mouth sores or dental problems.
Choosing a form with your clinician
If you only need help with erections, a swallowed tadalafil tablet has the strongest evidence and the most predictable absorption. Daily tadalafil through HealifyMD is from $99/month. If your clinician believes the blend is a fit, it is from $149/month. Both prices are confirmed at the eligibility review, with no membership fee.
Common questions
What is an ED troche?
A compounded lozenge containing one or more erection or desire medicines, designed to dissolve in the mouth.
Is a tadalafil troche better than a pill?
Not proven. FDA-approved tadalafil tablets have trial evidence; troches do not, and their absorption varies.
Why do blends with peptides come as troches?
Peptides like PT-141 and oxytocin are broken down by digestion, so pharmacies use routes that avoid the stomach. How well this works has not been established.
Does a sublingual ED medicine work faster?
It may for some ingredients, but onset also depends on arousal, dose and the individual. Tadalafil's long action is unchanged.
Are troches safer than tablets?
No. The same interactions and cautions apply, including the absolute ban on nitrates.
Can I chew or swallow the troche?
Follow your label, but troches are generally meant to dissolve slowly, and chewing or swallowing changes how much is absorbed.
Is the Triple Action Blend FDA-approved?
No. It is a compounded preparation and has not been reviewed by the FDA for safety, effectiveness or quality.
