Treatment comparison

Berberine vs. metformin

Berberine is often called a natural metformin. The two do share some effects on blood sugar, but they differ sharply in how they're regulated, how much we know about them, and how they're monitored.

Why the category matters

Metformin went through the FDA approval process: its makers had to show it works and is reasonably safe for type 2 diabetes, it's manufactured to drug quality standards, and its label lists dosing, warnings and interactions. Its use for prediabetes or weight is off-label, a decision a clinician makes based on evidence and your situation.

Berberine is sold as a dietary supplement. Supplement makers are responsible for their products' safety and labeling, and the FDA doesn't approve supplements before they're sold. That means two bottles labeled "500 mg berberine" may not contain the same amount, and there's no standardized label telling you about interactions.

How each one works

Metformin lowers the amount of glucose the liver releases, reduces the sugar absorbed from food and improves the body's response to insulin. Berberine, a compound found in plants such as goldenseal and barberry, is thought to work partly through an energy-sensing enzyme called AMPK, a pathway metformin also influences, which is where the comparison comes from.

What the evidence shows

Metformin has been used for decades, is a standard first-line medicine for type 2 diabetes and has been studied in large, long-term trials.

For berberine, the U.S. National Center for Complementary and Integrative Health says there is some evidence of a modest effect on blood glucose in people with type 2 diabetes, and that it may lower cholesterol, but there isn't enough rigorous evidence to say whether it helps with weight loss. A 2022 meta-analysis of 37 randomized trials in about 3,000 people with type 2 diabetes found berberine lowered HbA1c by about 0.6 percentage points on average. The authors noted that most trials were small, short (often around three months), conducted largely in China and of moderate quality, with some reporting implausible doses.

Side effects and monitoring

Both commonly cause digestive symptoms such as diarrhea, nausea, stomach discomfort and constipation. Metformin carries a boxed warning for lactic acidosis, a rare but serious condition more likely with kidney disease, heavy alcohol use, liver disease or severe illness, so clinicians check kidney function before and during treatment. Metformin can also lower vitamin B12 absorption over time, so periodic B12 checks are often recommended.

Berberine has no standard monitoring plan. It may interact with other medicines, it should not be used during pregnancy or breastfeeding, and it should never be given to infants because it can worsen newborn jaundice. Long-term safety data are limited.

The foundations matter more than either

Neither metformin nor berberine replaces the habits that do the heavy lifting for blood sugar in midlife. Regular movement, especially strength training and a walk after meals, better sleep, limiting alcohol and building meals around protein, fiber and vegetables all improve insulin sensitivity.

Knowing your numbers helps too. A blood test for HbA1c and fasting glucose tells you where you stand, and a short period of wearing a continuous glucose monitor can show how specific meals and routines affect you. Your clinician can use that information to decide whether medication is worth adding at all.

Don't combine or swap on your own

If you take metformin, don't replace it with berberine or add berberine without talking to your clinician. Combining two blood sugar-lowering agents can make results hard to interpret and may raise the risk of side effects or interactions with your other medicines. Likewise, if you're taking berberine and starting metformin, tell your clinician so they can plan accordingly.

Which might fit you

Your clinician will consider your blood sugar results, kidney function, other medicines and goals. These are general patterns to help you prepare for that conversation.

  • Diagnosed type 2 diabetes: metformin is a well-established, FDA-approved option; berberine isn't a substitute
  • Prediabetes or insulin resistance: metformin may be discussed off-label alongside lifestyle changes; ask about the evidence for your situation
  • You prefer a supplement: discuss berberine with your clinician first, choose a third-party-tested product and review your other medicines
  • Pregnant, breastfeeding or planning pregnancy: berberine should be avoided; discuss any medicine with your clinician
  • Kidney disease or heavy alcohol use: metformin may need special caution or may not be appropriate

Common questions

Is berberine a natural metformin?

Not really. They share some effects on blood sugar, but metformin is an FDA-approved drug with far stronger evidence and standardized quality.

Can I take berberine instead of metformin?

Not without talking to your clinician. Don't stop a prescribed medicine on your own.

Can I take both together?

Only if your clinician agrees, since combining them can complicate monitoring and increase side effects or interactions.

Does berberine help with weight loss?

There isn't enough rigorous evidence to say, according to NCCIH.

Does metformin lower vitamin B12?

It can reduce B12 absorption over time, so periodic checks are often recommended.

Is berberine safe?

It's often well tolerated short-term but can cause digestive upset and interact with medicines, and it should be avoided in pregnancy and breastfeeding.

How much is metformin at HealifyMD?

Metformin is $149 for 3 months ($50 per month), billed every 3 months, with any labs billed separately and your price confirmed at your eligibility review.
